Basic meeting minutes template (copy and paste)
This basic template covers what most business meetings need: details, attendees, decisions and action items per agenda item, and the next meeting. It is a simple word processing template in plain text: copy and paste it into any editor, then replace the brackets with the date and time, location, names and outcomes. Every line is customizable: delete what you don't use.

What is the proper format for meeting minutes?
The standard format runs top to bottom in the same order as the meeting: header, attendance, approval of previous minutes, one numbered entry per agenda item, action items, next meeting, adjournment and signature. The basic and formal templates on this page follow that order; the informal, action items and agenda formats trim it for lighter meetings. Seven formatting rules make it easy to read:
- Numbering follows the agenda. Agenda item 3 is entry 3 in the minutes.
- Decisions start with a "Decision:" label, so they stand out.
- Action items go in a table or on their own Action line, never inside a sentence.
- Entries read in past tense, like the Decision lines in the example above.
- Mark the document DRAFT in the header or footer until the group approves it and it becomes an official document.
- Name the file the same way every time:
YYYY-MM-DD [Body] Minutes — DRAFT, for example2026-10-06 Staff Minutes — DRAFT. - Budget 1–2 pages per hour of meeting, a working benchmark rather than a rule.
For readability, keep the page layout plain: one column, bold labels, header on top.
Which template should you use? The best meeting minutes template matches how your group decides. The five formats compare like this:
| Template | Best for |
|---|---|
| Basic | Staff, team and business meetings without formal votes |
| Informal | Quick weekly team syncs |
| Formal (Robert's Rules style) | Committees, associations and nonprofits that hold formal votes |
| With action items | Operational meetings judged by follow-through |
| Agenda + minutes | Recurring meetings with an agenda sent in advance |
Filling in the fields is a separate process, linked at the end of this page.
Formal meeting minutes template (Robert's Rules style)
A formal meeting minutes template is for committees, associations and nonprofit organizations that run meetings under Robert's Rules of Order: it records the quorum, follows the order of business and logs each parliamentary motion, a formal proposal put to a vote, with its result.
[Organization] — Minutes of the [Regular / Special] Meeting of the [Body]
Date / time / place:
Presiding: [name, title] Secretary: [name]
Members present: … Members absent: …
Quorum: [present / not present]
Call to order: [time] by [name]
Approval of agenda:
Approval of minutes of [date]: [approved / approved as corrected]
Reports: [officer / committee] — [received; any action taken]
Unfinished business:
New business:
Motion: "[exact wording]" — moved by [name].
Result: [carried / failed] ([X] for, [Y] against, if counted)
Action: [task] — [owner] — [due date]
Announcements:
Adjournment: [time]
Respectfully submitted, [name], Secretary Approved: [date]Four rules behind the fields:
- Quorum. Robert's Rules says it is never permissible to transact substantive business without a quorum, so the minutes show one was present; your bylaws set the number.
- Maker, not seconder. Record who moved each motion; the seconder's name stays out unless the assembly orders otherwise (MSU Extension, citing Robert's Rules of Order Newly Revised, 11th ed., p. 470).
- Voting. Give a vote count only if votes were counted; Robert's Rules doesn't count abstentions as votes (FAQ 6 on the same page), so list them only if your rules require it, and then as a separate number, never as votes against. In board minutes, a director's recorded abstention can matter for liability, so the board template gives it its own number.
- Signature. "Respectfully submitted" is a traditional closing that is no longer common, so keep it or drop it; what the record needs is the secretary's signature.
A board version adds executive session and conflict-of-interest lines (see the last section).

Meeting minutes template with action items
For operational meetings, decisions and action items matter more than the discussion, so this meeting minutes template starts with tables instead of narrative.
[Team] — Meeting Minutes — [Month D, YYYY]
Attendees: [names]
Carried over from last meeting
| # | Action | Owner | Due | Status (open / in progress / done) |
Decisions log
| # | Decision | Decided by | Date |
Action items
| # | Action | Owner | Due | Status (open / in progress / done) |
Notes: [only what explains a decision]
Next meeting: [date]Start with the carried-over table: open tasks get a new status, done tasks drop off. The decisions log records the outcome of decision-making, who decided and when, so settled questions stay settled. The action items table is where accountability lives: one named owner per row, never "the team," and a due date for each task. Project status meetings often add a Red/Amber/Green (RAG) status column and a risks table.

Meeting agenda and minutes template (combined)
A combined meeting agenda and minutes template is the meeting agenda you send before the meeting, with empty columns the secretary fills in during it, so there is no second document.
| Agenda item | Time | Presenter | Discussion | Decision | Action (owner, due) |
|---|---|---|---|---|---|
| 1. Approval of previous minutes | 5 min | Chair | |||
| 2. [Topic] | [10 min] | [Name] | |||
| 3. [Topic] | [15 min] | [Name] | |||
| 4. Next meeting and close | 5 min | Chair |
Fill in the first three columns beforehand and send the file as a pre-filled agenda. The Time column is a time box: when it runs out, the chair closes the item or defers it. During the meeting, the secretary fills in the last three columns, and the same file becomes the draft minutes, with no second meeting minutes template to fill in.

Is there a meeting minutes template in Word or Google Docs?
Yes. Word has free meeting minutes templates under File > New (search "minutes"), and Google Docs has a "Meeting notes" building block that pulls the event title, date and attendees from Google Calendar.
- Microsoft Word. Select File > New, type "minutes" in the Search for online templates box and press Enter (Microsoft Support). The same templates are online at Microsoft Create, including "Meeting minutes (simple)."
- Google Docs. Type "@", choose Meeting notes and pick a Calendar event (Google's announcement); the block also adds a Notes section and an Action items checklist. All blocks are listed under Insert > Building blocks > View more (Google Docs Editors Help).
The Docs block is a notes layout without quorum, motion or signature lines; paste the formal template above when you need them.
Every template here also comes as a file, no sign-up needed:
| Template | Google Docs | Word (.docx) | Markdown | |
|---|---|---|---|---|
| Basic | Make a copy | Download | Printable, ruled lines | .md file, Copy block |
| Formal (Robert's Rules style) | Make a copy | Download | Printable | .md file, Copy block |
| With action items | Make a copy | Download | Printable | .md file, Copy block |
| Agenda + minutes | Make a copy | Download | Printable | Not offered |
| Informal | Make a copy | Not offered | Not offered | .md file, Copy block |
Print the PDF as a meeting minutes form for handwritten minutes. The .md files suit wiki tools: Notion imports them under Settings > Import > Text & Markdown (Notion Help Center). Confluence Cloud documents Markdown as typing shortcuts (Atlassian Support), so check headings and tables after pasting.
How to fill a meeting minutes template automatically from a recording
If the meeting runs in a web browser — Google Meet, Zoom Web, Microsoft Teams Web or Webex — you can draft most fields of a meeting minutes template from the recording, while the formal fields stay with the secretary.
Meetlens is a Chrome extension that records the audio of a browser meeting without a bot joining the call, then produces a transcript with speaker labels and a structured AI report. Meetlens drafts the record: decisions, open questions and action items with owners and dates, and its timeline links each decision and task to its moment in the recording; you add attendance, quorum, motions and votes, and the signature. To fill minutes from a call recording, keep General Meeting, the default report format: it has Brief, Meeting Topics, Decisions, Tasks and Open Questions. Tell participants yourself: no bot means no recording notice from Meetlens.
For auto-fill, the General Meeting report and the meeting chat map to the basic template like this:
| Template field | From the General Meeting report or the meeting chat | Added by the secretary |
|---|---|---|
| Present | Meeting chat: "Who was present, and did the chair confirm a quorum?" | Surnames and roles, the spelling of names said only aloud, anyone who stayed silent and was never named |
| Agenda items | Meeting Topics (topics actually discussed) | Match to the official agenda and numbering |
| Discussion | Brief and Meeting Topics (gist of each item) | Trim to the detail your meeting needs |
| Decision | Decisions; meeting chat for who moved a motion and how members voted | Wording as agreed; in formal minutes, exact motion wording and vote result |
| Action items | The Tasks block: task, owner, due date | Check every owner and date against the recording |
| Open questions / parking lot | Open Questions | Choose which ones go on the next agenda |
| Approval of previous minutes | Decisions, when the group approves them aloud ("Approve the September minutes as distributed") | Corrections, if any |
| Adjourned | Meeting chat: "When did the chair adjourn the meeting?" | Check the time against the recording |
| Submitted by, Approved on | Not in the report | Fill in by hand |
The meeting chat covers the fields the report has no section for. After the meeting, ask it "Who was present, and did the chair confirm a quorum?" Pick the meeting with Add context so the chat looks only at it; the answer links to the moments in the recording it draws on and often quotes the exact words; play them back before you copy names into the Present line, because the chat knows only what was said aloud. On a staged committee meeting, it listed the seven members who spoke, plus the absent member and a silent guest because the chair named them, and it spelled the guest's name by ear (Li for Lee).
Then export the report as Markdown, TXT or PDF (Pro and Business plans) and paste its sections into your copy; Meetlens doesn't write into Word or Google Docs. This automation covers the first draft, not the record: the secretary checks names, figures and decisions against the transcript and, for formal minutes, adds quorum and vote counts, which the report doesn't capture; that check turns the draft into an accurate record.

What if you need more than a meeting minutes template, or less?
Beyond the meeting minutes template: writing up, board minutes, lighter notes
A meeting minutes template gives you the fields; the write-up and board rules have their own pages, and some meetings need less than minutes.
- Writing up. The guide on how to write meeting minutes covers what goes into each field before, during and after the meeting.
- Board minutes. A Board of Directors adds executive session, conflict-of-interest and resolution lines to the formal template; see the board meeting minutes template.
- Lighter notes. An informal check-in often needs only shared notes or a short recap email.
